Comparison of direct trocar insertion and Veress needle insertion

Aim: The aim of this study was to compare the effects of direct trocar insertion and Veress needle insertion on complication ratio, CO2 volüme and time consumption during laparoscopic procedures. Materials and Methods: 152 patients were included in this study which vvere operated forcholelithiasis laparoscopically in Selçuk University Meram Medical Faculty betvveen January 1998 - January 2001. The patients vvere divided into tvvo groups ran- domly. Group I: pneumoperitoneum was performed with direct trocar insertion after general anaesthesia, Group II: pneumoperitoneum was performed with Veress needle and trocar vvas inserted after general anaesthesia. Complication ratios and, time consumption and CO2 volüme vvere calculated in tvvo groups. Findings: Copmlication ratios vvere 11.8% in Group I and 13.2% in Group II, and the differences vvere not significant. CO2 volüme used insufflation and insufflation time vvere significantly more in group II than in group I (p<0.001). Conclusion: Direct trocar insertion vvithout pneumoperitoneum vvas superior than Veress needle insertion vvith respect of CO2 volüme and time consumption.
