If not pulled out, foreign bodies remaining inside bronchi for extended periods can cause complications such as bronchial stenosis, abscess and bronchiectasis. In our case, the patient was a chronic smoker who applied us having spontaneous pneumothorax. Bilateral giant bullae spreaded on lower lobes was spotted. Organized foreign body in the neighbourhood of throughly grown up bullae formation segment on distal part of right lower lobe was observed during the surgery. Agents of bronchial obstruction can cause proteolytic – antiproteolitik imbalance in check-valve mechanism and inflammatory process.
