Paget’s disease is the most frequent disorder of bone after osteoporosis characterized by excess bone growth due to increased osteoclast activitiy. It is usually seen in older males. Patients are often asymptomatic but sometimes bone pain, fractures, neurological and hearing problems can be seen. Although the pathogenesis of disease is not clear, genetical factors are thought to play role on pathogenesis. Paget’s disease is diagnosed by elevated serum alkaline phosphatase activity and abnormal radiological findings. Bisphosphonates are useful in controlling disease activity. Here, we present a patient admitted to our clinic with severe pelvic and back pain, mimicking sacroiliitis, diagnosed via bone sintigraphy as Paget’s disease in contrast to normal alkaline phosphatase levels.
