Gestational diabetes mellitus (GDM) was previously described as
any degree of glucose intolerance with first recognition at pregnancy
for many years. Currently, pregnant with DM at their initial prenatal
visit has been diagnosed as overt diabetes, not gestational diabetes.
The remaining pregnant who found not to have DM at initial visit
and not known to have DM should undergo oral glucose tolerance
screening at 24-28. weeks of gestation and diagnosed as GDM if
any value exceeds the threshold values. The prevalence of GDM
is reported 1-14% among the different populations. Its diagnose
is increasing over the world with new GDM criteria. Recently, ADA
has adopted the 2008 International Association of Diabetes and
Pregnancy Study Groups (IADPSG) conference criteria based on
the Hyperglycaemia and Adverse Pregnancy Outcome (HAPO) study.
IADPSG criteria for GDM is including; 75 gr glucose tolerance test
has been recommended as diagnostic test for GDM and a single
abnormal value has been considered to be sufficient for diagnosis
in this conference. The threshold values for glucose tolerance test
are as following; for fasting glucose; 92 mg/dl (5.1 mmol/L), for
1.hour; 180 mg/dl (10 mmol/L), for 2 hour; 153 mg/dl (8.5 mmol/L).
In this review, the modifications on GDM definition and the current
diagnostic criteria for GDM were evaluated.
